Prior to
your first appointment, information will be sent to your home or you can
print the forms from this website and bring them with you to the
appointment. Please complete this form prior to your
appointment. Also, bring a list of your 
current medication, any
allergies and any past medical history that you have.
We also
ask that you obtain any MRI and X-ray films and bring them with you to your
appointment. It is very important that you bring the actual films and
not just the report. Please bring your insurance card to your
appointment.
If you
have been treating with another provider for this problem, please have them
fax any medical records over to us at 302-623-4147.
At the appointment
the doctor will obtain a detailed history and physical exam. Questions
will be asked about the quality and location of your pain, factors which
worsen or alleviate your pain and other questions. The physical exam
will concentrate on motor and sensory function. The doctor also will
review all testing to help make any appropriate treatment recommendations.
